 Image of self relating to hardships

I was just wondering how all of you plan to handle students who have many hardships that contribute to their self-image. It is no secret that self-image is very indicative of performance in school and if a student does not perceive themselves well, then they may not perform well. If a student is developing a different sexual identity from their peers, a body-image disorder, experiences abuse at home, and there's a myriad of other things as well, how are we, as teachers, to handle such a situation. We want our students to succeed and we want them to be healthy. What to do if they won't reach out and we are not allowed to reach out?
